This system has been halted

HI gurus

 

I am in the process of installing 6.7U3 on a cisco B200 M3 E5-2650 No Ver and it seems like it does install but when is time to remove the media and reboot, I get the "this system has been halted" and the blade goes back to the version it was prior , I am trying to clean install 6.7U3

any help .

are you using CISCO custom ISO ?
Do you have enough free space on the disk or USB where the ESXi is installed to achieve the Update ?
