Converter fail at 97%

We're converting a physical server to VM using coldclone.

The server is an IBM xSeries 335, two disk RAID-1 (using hardware RAID) with Windows Server 2003 Standard Edition SP2

Converter started migrating all three partitions we have (C: D: and F:) but after finishing, it stops at 97% with error.

I'm attaching you the log files, who says: Task failed: P2VError UFA_LOST_CONNECTION()

We're connecting the server to the same subnet of the VirtualCenter and the ESX to avoid firewall problems (the physical server is originally connected behind a firewall)

Do you know any solutions to this problem? We're using Converter version 4.0.2 buid 119746

Have you tried also with VMware Converter 3.0.3?

Thanks Andre, I've read the KB carefully and applied some tips (use IP address of VC instead of machine name, don't resize partitions and don't modify the number of NIC cards)

All of them caused that VMware converter works without problems.
