I found the “configuration_error DENIED” on the Access log.
What mean is the “configuration_error”.
I would like to know what mean is “configuration_error”.

Dear Yoshinori-san,

I would recommend to perform a policy trace, because the exception will be argumented in the policy trace, allowing you to understand the reason why the exception is thrown. 
For example:
EXCEPTION(configuration_error): Authentication failed because of a configuration problem
