Thanks for your advise. We are not saving policy server trace log files, and no information in IDM logs.
And the error comes for few users only after they resetting passwords. we solve these issues by deleting the all SMAPS attribute values in user directory (i.e we are using CA directory for user store and policy and also provision store) by connecting with JXplorer.
We are suspecting these issues coming from cache or number of wrong attempts by the users.
What can be possible reason for this kind of errors.?
Our management having discussions on updating environment, upgrading process will start soon.

SiteMinder Policy Server trace logs will provide detailed information why the issues happen. It seems like that password blob data is not updated properly or user directory is not synchronized properly.
BTW, SiteMinder 12.5 is EOS version. I strongly recommend to upgrade into the supported version to get the official support.

Presently we are using CA Siteminder R12.5 integrated with CA Identity manager R12.6.
Some users are getting login page again after entering credentials though they changed password before password expiration.
Some users are getting blank screen after they entered credentials.For this we solve issue by deleting all SM values in user store.
What could be reason for this kind of issues in Siteminder environment.That we can give permanent solution.
