Service Catalog - Strange behavior on approval request

    I have an strange situation with Service Catalog.

    When I need approve a request, I do all steps and after approve the status remain "pending approval" with nothing on the history.

    If I try to approve again the same request it works... Which is really curious.

    We have now Service Management 17.2.0.5.

    Any Ideas about this behavior?
