What do you mean by "duplicates?" Do you mean identical first, middle and last names, but different other details? Because that could be a valid (although unusual) entry.
Or do you mean duplicate ID or UUID? Because that shouldn't happen according to the database restrictions.
You could backup the table, remove the duplicates and retry. But note that it appears to be failing on all types of tables in that log.
Does the original SDM 11.2 installation start the SDM Service WITHOUT leaving "ERROR"s in the stdlog?
You could also log a Support issue if you're moving to ITSM 17 after this, as we'd like to see you upgrade. :-) You'll still have legwork to do, but we may be able to give more pointers through a Support ticket.
