There is an vcenter 6.5 with "vCloud Director-1" extension installed.

It can be deinstalled via MOB: https://vcenter.local/mob/?moid=ExtensionManager&method=unregisterExtension

But after vcenter reboot, this extension is presented again!

    How to deinstall it permanently?

Yes, I did. Extension is successfully removed via MOB=>ExtensionManager=>UnregisterExtension=>InvokeMethod, but until next reboot only. After reboot, extension is presented again.

    Well that is pretty strange.

Dumb question, do you have vCloud Director installed and powered on yet?

No, there is no vCloudDirector installed and running.

Try to do the same procedure but restart the vSphere Web Client

Yes, I did it after extension unregistering: "/usr/lib/vmware-vmon/vmon-cli -r vsphere-client". But after reboot, extension is active again.

Which date do you have in "ExtensionManager"  >> "vCloud Director-1" >> parameter "lastHearbeatTime"?

lastHeartbeatTime of vCloud Director-1 extension is 2018-08-11T20:55:32.466999Z

Last boot time of vCenter host is 2008-08-11 21:33 (I rebooted vcenter immediately after vCloud Director-1 unregistering)

If you enter in the hyperlink of the parameter "server" in the same location you can see the url of the service. You can access that url?

Parameter "server" of type ExtensionServerInfo[] has value "Unset". It's not hyperlinked.
