Balancer went down with the following errors on console (was not pingable through all interfaces, only serial connection worked). Was not overloaded in the moment of failure (like 50% ASM CPU used) and worked fine a couple of years without any interruption before.

after upgrading up to latest ASR12400w, keeps rebooting

I would suggest to open a TAC with Brocade - if you have a active maintenance contract -
If no, i would try to reset / erase the ADX and reload a Image.
BTW, the error is not a Firmware DEFECT.
