I succeeded to reproduce your problem with a Quick delivery (QD) task with your scenario - both if you change the package manually on disk or edit it via UI.
The workaround is to make sure QD fetches the new package version is:
Go to your QD task and re-save it (change commandline-> save->revert back commandline-> save).
At least this worked for me.
If you would like to get an official pointfix for the issue please use your support channel to escalate the issue. Most probably the fix will be on the server side and you will not require to upgrade agents.,

What is your Altiris Agent version on clients ?
Is the issue about Managed Software delivery or Quick Delivery tasks or patch packages?
Do you have Peer-to-peer downloads setting enabled?

I updated a package with an updated vendor release. The package servers have them, but client PC's that still have the old version in its software delivery\ folder execute that version. I did do an update distribute points after updating the package.
