Allow users to add a description when inserting a new problem workflow task

Our Problem Management team would like to be able to add a description to any new tasks that they insert into a workflow on a problem ticket. This can be done after the task is inserted, however they would like to see the field on the insert new task form so that it can be done there. Their reasoning is so that they can add a description of what needs to be done for this particular task to the assignee of the task.

Yes, this idea is being considered for all ticket types.

Current functionality allows to insert assignee, group, CI and dates.

The idea is to allow insert task description, so you can define what should be done
