Yes already enabled SSO with SAML for all the product but still we are not able to see Catalog form in Service Point.
Let me ask you few question,
1. Which version of CASM you are using?
2. Which type of SSO you are using?
3. Are using using HTTPS or not?

I had similar issue, Catalog icons are showing in Service Point, but when you hit the icon to open Catalog form then error pops up, and you can see the error in Console.
Resolution to mine issue was to enable SSO on CA Service Catalog in the Administration Tab in Catalog.
Restarted Catalog service and it works fine.. When SSO is enabled in xFlow/SP then you need to allow it also in Catalog.

What should be contentSecurityPolicy setting in Service Catalog web.xml($USM_HOME\view\webapps\usm\WEB-INF\web.xml) to see the service catalog form from Service Point.
Below is the out of box settings for contentSecurityPolicy in web.xml
