Its possible to create relationship  Configuration Itens x Symptom_code.

I need to custom that show only symptom_code that have relation with any configuration Item.

I created a custom table to show relation between (CI x Symptom_code)

How to proceed to custom form symptom_code to made relation wirh CIs?
