Currently I am counting the total number of User Stories per Iteration and dividing that by the number of Defects found in that same Iteration and this isn't giving me an accurate Defect ratio per Iteration  Is there a report that would only look at User Stories that have QA Tasks assigned to those User Stories?

I saw a custom app that can show stories with tasks, but not sure it was limited with only QA Tasks.  How are you getting the counts of defects / stories per iteration currently?

Unfortunately it's a fairly painful process.  From the Track tab, I  select Iteration Status and take the total number of User Stories and divide that by the number of defects in that Iteration to gather the defect ratio. Unfortunately just taking the total number of User Stories and Defects isn't an accurate percentage because not every User Story may include QA testing. I am tracking this for multiple teams and have been unable to find any type of report that suits what I'm looking for.
