Need a gel script to update the approved by and approved date attributes in Risks,Issues, Changes-> change requests processes

In Projects->Risks,Issues,Changes -> Change Request. When a new change request is created and approved through a process, the approved by and approved dates aren't automatically populated. 
This couldn't be done through the set of actions in a process. Any scripting which could through the approved by and approved date automatically in the change request would be helpful..

Need to retrieve the approver and from the previous action and then implement in the script.
