Identity Portal - Custom form that link to execution plan

Hi Team,
Q. It is possible for us to link a custom form create in Identity Portal to execution plan ?

Due to some limitation of using "Access Module", we plan to create a custom form that will handle the "Access Request" from End User.
Customer currently have target permission/prov role that required different level of approval. 
eg. some target permission required 1 level approval, some target permission required 2 level approval. 

We already have solution(execution plan) on how on how to handle 1 level or 2 level approval in target permission.

Question here is that, if we want to build our own "Access Module" using custom form, is that possible ?

We haven't tired this yet.  Perhaps you can test on your end and let us know.

What are you trying to achieve with custom form? Please explain how is it your custom form and how do you want to associate the custom form with execution plan?
