Storing Form Field Values from CA Service Catalog within a Process Automation Dataset

Hello ... I created a custom form in Service Catalog and want to send the data to a Process Automation process and store the data within a dataset.  Could someone point me to some documentation that would explain that process step-by-step?  I took some CA courses for using PAM with Service Catalog and Service Desk, however, the instructor didn't touch upon that particular subject.  Thank you.

Check out some of the processes that were populated into PAM when you integrated Service Catalog. Look in the CA SLCM folder. GetFormFieldsByRequestID gives an example.
