There were multiple versions and service packs and cumulative releases that came out between r12 and the current r12.52 sp01 cr04.
You will need to go to each hot fix or release and see what fixes or functionality were added with each.

Product Pages
On the right hand side go to Hotfix ->Cumulative Release and then select Web Access Manager which is the policy server. You can then select which CR or SP you are interested in and get Solution Document RTC information for each fix.
